ERROR, MyAchievements Profile

It only happens in one persons profile? Or cetain people not sure.
It shows up at the top of the screen. earlier it was 2 messages.

Then i uninstalled it and reinstalled it started from scratch all over.
The reason i did that was because peoples rewards we ending up missing
and kept having to re 'give' them! Please help guys.. And also the post
count achievements were not giving out automatically like they were
suppose to so i decided to try over...

So now that i reinstalled myachievements. and added a goldbar
award for post count.. everyone got the goldbar.. expect this guy
well that i know up.. and when you goto his profile you get that message
Also in his userid profile.. The post count award/achievement is there..
but its a broken img.... BUT only broken on HIS profile not everyones.
